Lowering the Episiotomy rate with Epi-No Birth Trainer in Israel
For this study, we attempted to contact all the women (there is only one
distributor in Israel) who bought Epi-No Birth Trainer in Israel from
October 2000 - October 2001 and gave a working phone number upon purchasing it
(Total = 164). We asked the following questions on the phone:
1.Did you use it more than 3 times? (Inclusion criteria of previous study
(6)).
2.Was it your First birth? /Second birth? / Third birth?
3. Was it a spontaneous birth or Instrumental or C/S?
4. Did you have an Epidural?
5. Gestation age in weeks at delivery
6. Birth weight of the baby
7.Did you have an episiotomy? / tear? / stitches?
Using Epi-No, the women who delivered a full term vaginal first births
in hospital settings in Israel (Total = 142) had a episiotomy rate of 19% (27),
12% (17) had second degree tears which needed repair,
53.5% (76) had absolutely no tears or stitches at all, and 15.5% (22) had small tears
that were repaired with a "one or two stitches".
One woman was excluded because she had twins, 2 were excluded because they
practiced less than 3 times, 1 was not pregnant, but using it to strengthen
her perineal muscles. 4 women (3%) had cesarean sections. None of the women
(Total = 14) who used it for second or third births had an episiotomy
although they bought it because of previous trauma from episiotomies.
None of the women reported vaginal infections at time of delivery or vaginal
infections resulting from the use of Epi-No. The average birth weight was
3315 grams

Most of the women recommend the use of Epi-No after their experience. Only 3
women were negative about Epi-No. One said that she is not sure if it helped
and that it hurt alot to use it. Another said that it was not nice to use it
and it didnt help (epidural/vacuum/big episiotomy). The third said that it
didnt really help (Episiotomy and tear, 2950 gram baby)
The rest said things like:
"It was the best investment of my life."
"It gave me confidence."
"It taught me how to push".
"You see progress, you learn to exercise the muscles"
"Happy"
"Delighted"
"Helped alot"
"Wonderful, I am sure it helped me alot"
"Worth all the effort"
"Sorry I didnt use it enough!"
"Helped the recovery after birth"
"I had bad stitches twice and on this birth -none!"
"I had a VBAC with no episiotomy! I am very happy!"
Several women who had episiotomies said "I know it helped. I didn`t really
need the episiotomy. The midwife did not know any other way."
Many women told their midwife not to cut because they were using it and
at one location (Ichilov Hospital), some of the midwives had actually had
heard of it and tried to obey the woman`s request.
All the women who used it after having traumatic episiotomies on first and
second births were happy with the results- None needed episiotomies.
Several first births reported 15 minute pushing time which is remarkably
fast for a first birth.
