טרנספורמציה בלידה - Transformation in birth

 I was a doula at a Hospital birth to an unprepared primigravada woman. Her birth was augmented with Pitocin and she was on her back, out of control in pain and screaming, demanding a cesarean.

הייתי דולה ליולדת בלידה הראשונה שלה בבית חולים. נתנו לה זירוז ומצאתי אותה שוכבת על גבה כואבת צורחת חסרת שליטה ומתחננת לניתוח קיסרי.

Despite the language and culture barrier, I made her my friend. We began laughing and singing in the rest time between contractions. I modeled for her how to move and got her out of bed. I showed her how to lunge, how to sway her hips around, how to use a birthing ball and keep moving, which she did throughout, even though she had some confinement on her movements due to IV and continuous electric fetal monitoring.
על אף פער של שפה ותרבות התחברנו. התחלנו לצחוק ולשיר בזמן מנוחה בין צירים. הדגמתי לה כיצד לנוע ועודדתי אותה לצאת מהמיטה. הדגמתי לה כיצד לזוז הצידה עם רגל מורמת ומכופפת על כסא איך להניע את האגן בתנועות סיבוביות
איך להשתמש בכדור לידה ולהמשיך להיות בתנועה והיא התמידה לכל אורך הצירים למרות המגבלות שיצרו עמוד האינפוזיה ושני חגורות המוניטור הרצוף

It was a profound experience for me to see her transform from a passive victim in a frightening process into a woman who has slowly found and become in touch with her inner wisdom. Between contractions she kept changing positions. During them she became quiet and  went deep inside  herself. She had a beautiful baby boy in a normal vaginal birth.
זאת היתה עבורי חוייה חזקה לראות את הטרנספורמציה שלה מקרבן פאסיבי בתהליך מבהיל לאישה אשר באיטיות גילתה והתחברה עם החכמה הפנימית שלה. בין צירים היא שינתה תנוחות ובתוך ציר נהייתה שקטה ונתכנסה עמוק בתוך תוכה. היא ילדה תינוק זכר יפה בלידה ואגינלית נורמלית..

I stayed to help with breastfeeding and left with a feeling of purpose and satisfaction.
נשארתי לעזור בהנקה ועזבתי בהרגשת סיפוק ומשמעות.

I do indeed love the opportunity to cross all sorts of bridges as a doula. Looking back, It was so preposterous and unexpected that the laboring woman and I totally understood each other despite any logic. We shared no common language. We were on opposite extremes of the religious, cultural and political spectrum. Communication should have been impossible. She had not had access to a child preparation class. She should not have learned how to move with her contractions so quickly. Transition ought to have been the absolute worst, but she was at her calmest. How could it be?
אני באמת אוהבת את ההזדמנות לחצות כל מיני גשרים כדולה. במבט לאחור זה היה בלתי אפשרי ובלתי מצופה שהיולדת ואני הבנו אחת את השנייה בניגוד לכל הגיון. לא היתה לנו שפה משותפת. היינו בקיצוניות הפוכה של דת תרבות ופוליטיקה. תקשורת הייתה צריכה להיות בלתי אפשרית. היא לא נחשפה להדרכה לקראת הלידה. היא לא היתה אמורה להתנועע עם ציריה ולקלוט את הענינים כל כך מהר.  בין שבע לעשר ס"מ היה צריך להיות החלק הכי קשה לקראת פתיחה מלאה אבל דוקא אז היא נהיתה שליווה מתמיד. הכיצד.

We already know from research the uncontested significance of having a doula during labor and birth. Having ongoing emotional and physical support at this sensitive time affects birth memories and outcome and has been shown to decrease pain and need for medication. Yet in my experience I felt I was seeing something new; Apparently, the Doula transcends language and culture. How awesome is that!  She spoke no English and I, no Arabic. We did actually share a language; the universal and unifying language of childbirth. Transition was easier because of the climate of trust that allowed her hormones to flow. Her endorphins were having a party. The birthing mother and I put aside everything society has taught us to believe defines and diversifies us. We found a deeper layer of being, an essential core of strength and inner wisdom.
אנו כבר יודעים ממחקרים ההשלכות הבלתי מעורערות שיש לדולה על מהלך צירים ולידה. תמיכה פיזית וריגשית רצופה בזמן רגיש זה משפיע על זכרונות ותוצאות לידה ומפחית תחושות כאב וצורך במשככי כאב. אך בחוויה שלי הרגשתי שאני עדה למשהו חדש-כנראה הדולה מתעלה על שפה ותרבות. כמה זה עצום! היא לא דיברה אנגלית ואני לא דיברתי ערבית. למעשה גילינו שפה משותפת- השפה המאחדת והאוניברסלית של לידה. בין שבע לעשר ס"מ היה קל יותר בגלל אקלים הבטחון שאיפשר להורמונים שלה לזרום. האנדורפינים שלה חגגו. היולדת ואני הנחנו הצידה כל מה שהחברה לימדה אותנו שמגדיר אותנו ומבדיל ביננו. מצאנו שיכבה יסודית יותר של הווייה ליבה חיונית של עוצמה וחכמה פנימית.

Touching that essence is another reason to embrace the natural un-medicated birth. That is what the woman who has been socialized to associate birth pain with fear misses, when she takes an epidural with her first contraction. That is what is lost when technology, which is used unnecessarily or routinely, robs women of their right to their initiation rite into womanhood. Pain with a purpose allows the hormones their rightful stage center place in the birth story, and liberates the woman to connect to her deeper essence. At that fountain, there is only one language, one culture, one simple story of mother and baby
לגעת בתמצית הזאת זאת סיבה נוספת מדוע לחבק לידה טבעית ללא משככים ותרופות. זה בדיוק מה מפסידה האישה שלמדה לקשור כאבי לידה עם פחד כאשר היא לוקחת אפידוראל בציר הראשון. זה מה שנפסד כאשר טכנולוגיה אשר מנוצלת ללא צורך רפואי או באופן שיגרתי גוזל מנשים זכותם לטכס מעבר לנשיות. כאב עם משמעות מאפשר להורמונים מקומם המרכזי הטבעי בסיפור הלידה ומשחרר את האישה להתחבר למהותה העמוק יותר. במעיין ההוא יש רק שפה אחת תרבות אחת סיפור פשוט אחד של אמא ותינוק

 It is at this fountain, in this primal birth experience of women who come face to face with their power, I believe we may find the wellsprings of reconciliation and healing which can transform us individually and as a society. How does it transform us individually?  In childbirth, the mind is at peace with the process of labor and does not resist, allowing contractions to intensify and be more effective.  By re framing personal challenge and pain as our ally instead of our foe, we stop wasting energy fighting against ourselves and find dynamic and creative solutions.
במעיין זה בחויית לידה ראשונית של נשים הנפגשות פנים אל פנים בכוחן אני מאמינה נמצא בארות התפייסות והחלמה שישנו צורתנו כאינדיוידואלים וכחברה. כיצד זה משנה אותנו כבני אדם? בלידה המוח בשלום בתהליך הלידה ולא מתנגד וכך מאפשר לצירים להתגבר ולהתייעל. כאשר נמסגר מחדש אתגר אישי וכאב כבעל ברית שלנו ולא אוייב אנחנו מפסיקים לבזבז אנרגיה בקונפליקט נגד עצמינו ומוצאים פתרונות דינמיים ויצירתיים.

How can a mass of women having positive births transform us collectively?And wouldn't that achievement be enough? Yet the former makes collective change inevitable. She has not only passed a critical event in a less stressful way. She is altered, her perception is altered. Imagine if we could apply to all of society the same perceptual re framing, and transfer the lessons of child labor to global economics, to politics, to water food and oil shortages. What if nations could perceive their struggles as universal forces to be reckoned with instead of perceiving them as threats to be coped with by fighting to destroy one another? What changes would we see then?

I see women as the grassroots inspiration for this silent revolution of acceptance, dialogue and initiative, diversity instead of adversity, sharing resources to meet human needs instead of squandering them, educating for rights instead of feeding ignorance and fear.
אני רואה נשים כהשראה מהשורשים למהפיכה השקטה לקבלה דיאלוג ויוזמה שונות אבל לא יריבות שיתוף משאבים לספק צרכים אנושיים במקום שאחד לוקח על חשבון אחרים חינוך לזכויות במקום הזנת בורות ופחד

If we can educate the next generations of women to stand up and claim their right to positive and powerful birth experiences, embracing their emotions, needs, rights, and common goals,  a generation who seek to encounter pain with curiosity and openness to learn, we may live to see a better world replacing despair with initiative and replacing armed struggle driven by harted for cooperation and collaboration.
אם נוכל לחנך את הדורות הבאים של נשים לעמוד לדרוש ולקחת זכותם לחוויות לידה מעצימות-אם נשכיל לבנות דור נשים מקבלות רגשותיהם צרכיהם זכויותיהם ועושות למען מטרות משותפות - דור המסוגל לפגוש כאב עם סקרנות ופתיחות ללמוד אולי נחיה לראות עולם טוב יותר המחליף נואשות ביוזמה ומחליף מאבק מזוין מונע משנאה בשיתוף פעולה ועשייה משותפת.
