Hi Michaela, If the babies head is too big, then it won't engage in the pelvis, or will not be able to maneuver through the bony pelvis. This is not related to tearing, and usually will require a Ceserian. Sometimes it is not too big, but is not sitting in the perfect position and may need to be turned by vacuum or forceps. If the head is very big, sometimes there is tearing in the vagina or perineum or labia (lips). Tearing has a lot to do with your bodies tissues and how they stretch, and how big the head is and the shape (wide forehead or long cucumber head). However, you can help to prevent tearing by using the epi-no during the last month to help do passive stretching, and if the midwife helps you with warm compresses and oil and a slow delivery of the head while you are breathing it out from the point of crowning, while she helps to keep the head flexed thereby keeping the smallest possible diameter. You really should use the epi-no now, since you have to deal with scar tissue from your last birth due to the cut. Best of luck.
