Very infrequently, there is some bleeding with the Epi-no training, and here are the usual reasons. Sometimes, it doesn't have to do with the Epi-no, but is coincidental with losing the mucous plug. Although usually the plug is more mucous consistency, sometimes it is a few drops of clear red blood, a few times a day, a spotting. Since you are 39 weeks, this is possible. Sometimes women have "left-overs" from their hymen, called hymenal tags, which get torn during birth or use with Epi-no. A more common cause of bleeding is when the Epi-no gets inserted more than 2/3 of the way deep into the vagina, inflated big (like 9), and then when it is removed, you get a small laceration because the tissues at the entrance to the vagina have not been properly stretched. By deeply inserting the Epi-no in the vagina, you are stretching the middle of the vagina, and not the entrance, since the widest part of the balloon is the middle. That is why you need to only put in half or 2/3 of the balloon. This is not dangerous, and doesn't require stitching, and will heal in a few days, just like small birth lacerations do. But you need to stop using the Epi-no and let it heal until the birth, or at least a week. It is also possible that your skin is not healthy due to a candida infection. This would cause burning and redness and heavy discharge. For now, I would rest with the epi-no, as you have stretched until 9, which is very good. If you don't birth in a week, you can try again, but slowly. I would like to address the woman who questions why we promote natural birth but suggest the Epi-no. I for one, suggest ways to birth that are advantageous, not necessarily old-fashioned or totally natural. For example, I don't suggest birthing at home without skilled professionals like midwives to be present to identify or assist with any problems. I say "yes" to natural birth over drugs and epidurals because it is healthier for the baby and the mother. I don't say "no" to antibiotics when there is infection, or not ever to use an IV or Pitocin to stop bleeding after birth. I don't say that you don't need to listen to the baby's heartbeat and just rely on nature. The same with the epi-no. I have seen the huge difference on the rate of tearing with women using and not using Epi-no for first births. I say if you don't tear and need stitches because you preparted with a "modern technologic" device, so you will have a better birth, than if you tear, have bleeding , need stitches, and have to recover and heal.
