Hi Salush,
From the sixth birth onwards, supposedly risks of bleeding are higher due to a "overused uterus", although I personally don't see this. Perhaps because I make sure that these women nurse immediately after birth and they are experienced nursers so the babies latch right on the nipple. Anyway, I also massage the lower abdomen after the placenta is out which helps to keep it contracted and prevent excessive bleeding. If there is doubt, then I give a shot to contract the uterus. In all hospitals this is routine for all births, to give pitocin or syntocinon or methergin after the placenta is out. This causes strong uterine contractions which can be painful and is usually not necessary. Perhaps it has become such a routine in ho;spitals because they take away the baby for weighing and dressing and shots instead of letting him nurse after birth, and also because there is not one to one continuous care to observe for excessive bleeding. I always carry this with me for problems but only give it if necessary, and it is unusual to need it.
