It's been quite a month for me. Last month was wonderful with a bunch of great homebirths, mostly quick and easy, and putting me on a real high. This month so far is a real test of faith. Two of my first birthers stayed breech, despite the exercises, and moxa, and failed external version היפוף חיצוני . They ended with Cesarians. Then a lovely lady, first birther, at 42 weeks had a very long labor, taking 3 days to get to 3 cm, with lots of contractions but irregular, then 12 hours to get to 5 cm, despite trying homeopathy, acupuncture, and the breast pump. She just couldn't get into the active phase with good strong and hard contractions. She was exhausted but never lost hope. I finally suggested they go to the hospital for labor acceleration with Pitocin זירוז because I just couldn't help them anymore. There in the hospital she got an IV with Pitocin and dilated to 7 cm. then the heartbeat began to drop so they had to stop the pitocin and then she got an epidural, and just didn't progress, despite the head being low all the time. In the end she had a Cesarian and birthed a sweet baby of 3800. Then in recovery she began to lose a lot of blood, and in the morning they had to take her back and reopen her and find the source of her bleeding, as she was in a very bad way and her life was threatened. Her poor husband even was asked to sign a form permitting hysterectomy (כריתת רחם) if needed to save her life. Lucky that this was not necessary and her uterus was saved. She developed lots of blood clotting problems and was in very critical condition for the next 2 days, but now is fine and recovering nicely. Then I had another first birth, that went very well and quickly as far as the dilation went, but then she pushed for 3 hours with the head appearing at the lips for nearly an hour with no progress. I was fearing a big baby, and decided that it was necessary to transfer her also. At Kaplan Hospital, they were very nice, and we fell into the lucky hands of a midwife friend from Misgav Ledach, Raya, who was wonderfully helpful and encouraging. The doctor was also nice, and we gave her a little pitocin and the doctor agreed to wait 10 minutes and if she didn't birth she would have to do a vacuum. With a lot of help from all of us and incredible pushing efforts from the woman and a drop of Pitocin, the baby was born, big but not so big like expected (3830) and the Ultrasound a week ago said 4100. Only a small tear with a few stitches. There was a lot of thick meconium that came out after the head, and the baby had to be suctioned a lot, which later on disturbed the breastfeeding efforts. Next day mother and baby were home, but the breastfeeding is not going so well and she is being helped by a lactation consultant. For me, I know that I had to transfer her and I am lucky that it ended in a normal birth, but it is very frustrating not to be able to birth her myself and continue in the hospital. Now, I have someone in labor , a third birth, with two previous unsatisfactory emotional birth experiences, and she is now in early labor and doing very well. I just checked her again, and she is now 5 cm, and went into the jacuzzi so I feel really very optimistic that this is going to be a wonderful and fabulous birth and I am soooooo ready for a great experience after this month of stress . So now I must go and attend to her needs and help her make this a very positive and fulfilling birth experience for her. I will keep you all posted as to the results.
